Working Papers

Students interested in obtaining their Working Papers must download the Working Papers Application as well as the Physical Fitness Form below.  Students with an up to date physical with the school nurse (no older than one year from date the application is submitted to the Counseling Center) can have the school nurse complete the Physical Fitness Form.  If there is no physical or up to date physical with the school nurse, students may schedule a physical exam with the school nurse or see their own doctor to complete the form.

Please return the completed application (Part 1 ONLY) and the completed Physical Fitness Form to the Counseling Center.
