Seniors,

Below you will find several forms that you will be required to submit to your counselor for various purposes.  These forms are intended to facilitate the Counseling Department in submitting important college application documents as well as other graduation requirements.

Diploma Name Form - Diploma orders are being processed.  Names will be printed on diplomas as your name is listed on your Power School account.  If your name is incorrect in Power School or you would like to add a middle name or middle initial to your diploma, complete the Diploma Name form.  If you are changing your name, you must provide your birth certificate so that the changes can be updated in Power School.

The deadline to complete and return this form to your counselor is December 1st.
